Clinical Care Coordination
At The Makeda Collective, we understand that lasting recovery often requires professional clinical support, whether it's trauma healing, mental health stabilization, or medication management. We offer clinical care coordination: personalized guidance to help each client access the right licensed professionals for their unique needs.

What Is Clinical Care Coordination?
Clinical care coordination means we help clients and their families identify, access, and stay connected to trusted providers, which includes therapists, psychiatrists and IOP programs. We work with existing treatment teams when they’re in place and we can also build a care team using our vetted network of trauma-informed, recovery-aligned clinicians. Our role includes helping with referrals, IOP enrollment, insurance navigation, appointment scheduling and ongoing collaboration with all involved providers (with client consent).

Benefits of Clinical Care Coordination
-
Matches clients with providers who understand recovery and women’s mental health
-
Supports enrollment into appropriate IOPs, therapy or psychiatry
-
Reduces confusion by helping navigate options and logistics
-
Encourages consistency across all areas of care
-
Integrates clinical support with mentoring, housing, and case management
-
Provides families and providers with peace of mind through clear communication
-
What We Offer
-
Individualized assessment of clinical needs with in-house, clinical oversight
-
Referrals to therapists, psychiatrists, IOPs and other providers
-
Support with insurance, scheduling and intake paperwork

Regular follow-up to support treatment engagement and integration

Healing happens best when every part of the support system is working together, intentionally and collaboratively.
